Fasting is not required for hormones, but we recommend having your blood drawn in the morning between 7 and 10 o'clock, testosterone is at its highest then and most suitable for interpretation.
Total testosterone measures all testosterone in the blood. Free testosterone measures the active, unbound fraction. SHBG partly determines the ratio between the two. The combination gives a more complete picture than total alone.
With testosterone replacement therapy, the values mainly reflect the medication. Please let us know so we can adjust the interpretation accordingly.
